[mdx] Concepts document V1.9

I have finished fiddling with the Concepts document for now.  I think  
I have managed to incorporate all of the notes I took during the  
Arlington meeting, including managing to shrink the document by a page.

Rather than trust the list software, I've put up a temporary web page  
here:

	http://web.me.com/iay/mdxtemp/

You should be able to access either a clean PDF from that page, or a  
version with changes highlighted.  You probably want both, though.  My  
apologies in advance to anyone who is using a browser that gets cross;  
please let me know if that happens and I'll send you a separate  
copy... this was just the quickest way to slap a couple of files up  
there.

If you have time over the next couple of days, please take a look and  
let me know about any obvious howlers, or things you think I forgot to  
write down and fix.  I have another couple of pages of notes written  
down for the *other* document, of course...

My current plan is to wait a couple of days for comments and then  
publish the final version of this as an attachment to a post on my blog.
