[mdx] syntax for entity-attributes

Oh dear, I'm afraid I wasted a lot of bandwidth. I'm talking about this:

(1) '{' name-format '}' name '=' value
(2) name '=' value
(3) value

Case (2) defaults to the following value for name-format:

urn:oasis:names:tc:SAML:2.0:attrname-format:uri

Case (3) defaults to the following value for name:

urn:oasis:names:tc:SAML:2.0:nameid-format:entity

As I said, I don't see how nameless tag values fits into this syntax at all.
